The course introduces the full RCA process, including how to select the right events, organize an effective RCA team, and apply analytical tools that reveal underlying causes rather than symptoms. Attendees will work through practical exercises using methods such as 5 Why, Fishbone Diagram, fault tree analysis, sequence of events mapping, and change analysis. The focus is on building strong problem‑solving skills that translate directly to daily work.
Participants will also learn how to document findings, communicate results, and sustain improvements through tools like project charters, A3 reports, and audits. Whether your team is new to RCA or looking to strengthen existing practices, this training provides the structure, clarity, and hands‑on experience needed to drive meaningful improvement.
